We borrowed this form from W3C.org just to show that forms can be manipulated to be used with Responsive Design. W3C is a great source for tutorials and reference guides. Be sure to begin right by using the W3C validator to check your html and css before posting online.

You can actually use the form by changing action="#heading" to action="feedback-form.php" and making any changes needed to the fields.
